The Surgeon is a hostile mad scientist whose headquarters are located in the CEO Offices of the Red Racer Tricycle Factory.
He/she has been experimenting in mind control by implanting chips into multiple Ghouls and Super Mutants inside the factory. There is a fail-safe built into the chips which will cause all the devices to explode if a signal is sent from a computer terminal in the office. If your hacking skill is high enough, you can initiate this control and consequently kill all the creatures in the Red Racer Factory (except for the caged Glowing One ghoul known as Stefan).
The Surgeon has health on par with a heavy-weapons Raider and carries a Missile Launcher and a key that unlocks Stefan's cage and two nearby safes. If you shoot the weapon out of his/her hands, he/she will automatically spawn one of several alternative weapons in random order (including the Sniper Rifle, Chinese Assault Rifle, Sawed-Off Shotgun and Scoped .44 Magnum, along with ammunition for each).
It's worth noting that he/she wears a rather pristine Vault Jumpsuit under his/her lab coat, indicating that he/she might be a former vault dweller. However, the game does not allow the player to remove the lab coat and, since no identification information is displayed through the Pip-Boy, the vault number he/she came from is not identifiable.

- The Surgeon does not appear to have a fixed gender as players have reported observing the character as both female and male with a random appearance.
- If you wear the Ghoul Mask, The Surgeon will be friendly to the player and can even be spoken to (though he/she does not have much to say), due to having the FeralGhoulFaction on them to make him/her non-hostile with the enthralled Ghouls.
- The Surgeon will drop a Finger if the player has the Lawbringer Perk, even though his/her karma level is neutral.
