This note was meant to be added to the Lone Wanderer's Pip-Boy as a note from Moira during the first chapter of the Wasteland Survival Guide quest. This can be seen from the editor id MS03 which links it to this quest. One of the tasks is first reaching Minefield's playground, then obtaining frag mines.
