To deal with this, save your game before trying to pick up the guide. Then Approach slowly and just target the book with your crosshairs properly and click your button to fetch it in one hit. Don't try to pull it somewhere and it should be no problem. Or just try to walk into a spot where you can reach it and it reappears and simply take it.

Upon completing the combat, you will notice one of your followers is now missing, but without a message that they've died or been knocked unconscious. This has happened with both ED-E (while also traveling with Veronica), as well as Arcade Gannon. Gannon returned via the Vault 22 Elevator "trick", however ED-E did not, though is still considered in the party.
