- This might be one of the most difficult perks to use, since throughout the game you lose and gain Karma which easily changes your general status.
- One way to control Karma and keep it neutral is to complete quests in the most neutral way when possible, or the positive way when there is no neutral option, then stealing small items or repeatedly accessing an owned terminal to drop the alignment back down to neutral. Or, performing an evil action in a quest then giving purified water to someone such as Carlos outside Rivet City or donating caps to a church such as Saint Monica's Church or the Church of Atom.
- Karmic Rebalance can help players who are having difficulty maintaining a neutral status (though the Karmic realignment is just a one-time bonus).
- When speaking with Justin during the Blood Ties quest, this perk lets you bypass a speech challenge altogether when you have neutral Karma.
