A headshot can refer to two things:

• A single, well aimed shot to the head which kills the enemy and can blow off their head.
• A general shot to the head.

The first reference is regularly used when Sniping, in which the Sniper Rifle is powerful enough to kill a enemy in a single headshot. On Fallout 3, with high enough Small guns, Big Guns or Energy Guns stats, you can completely destroy the enemy's head or decapitate them.
