I play on Xbox 360, and I seem to get the Caution sign overhead with no enemies anywhere. This has happened numorous times, and I cant find any way around it. Any ideas? $Insert formula here$

I know it sucks when that happens. This game has quite a few bugs on the ps 3 console. The solutions I know that work for the PS 3 version are:

logging in with a new profile and then relogging.
Fast travelling or entering buildings.

Restarting your system. Reloading from a previous file. Saving... and then reloading. Clearing cache may work?

Hope this helps. That can be annoying
