Backstabbing is a skill that was to appear in the canceled Fallout: Brotherhood of Steel 2, skills in this game work like perks do in other Fallout games.
Backstabbing allows the player character to perform special stealth-based attacks, specifically maneuvers with bladed weapons. This skill was added as part of a new "Sneak Mode" and is unique to Fallout: Brotherhood of Steel 2.[1]

Fallout 4
In Fallout 4, Backstabbing is an in-game combat stat. Backstabs are stealth-based kill animations achieved by attacking an enemy with a melee weapon, while undetected, and dealing enough damage to kill the target.
- The stat will not update in the Pip-Boy unless done in V.A.T.S.
